Dr Victor Sammut has looked after the health needs of the local community for more than 25 years and has built a strong reputation for providing patients with thorough health care.
Dr Sammut is devoted to the pursuit of better health and well being for all patients and amongst his special interests includes: asthma, diabetes, hypertension, kidney disease, chronic disease management, depression, musculoskeletal treatment and preventative medicine.
Dr Sammut speaks English, Maltese and Italian and enjoys walking, singing and spending time with his family and friends.
Dr Azin Malekzadeh
Dr Azin is an International Medical Graduate working towards her fellowship with the Royal Australian College of General Practice under the supervision of Active Medical Principal GP Dr Victor Sammut.
Dr Azin has a special interest in women’s health, children’s health, immunisations, mental health, chronic disease management and travel medicine. Being a mother herself, Dr Azin has a keen interest in supporting parents and children and giving families the best chance at being healthy.
Dr Azin enjoys walking, listening to music, travelling, spending time with family and friends and offers bilingual consultations to patients in both English and Persian.
Dr Jae Mie Teh

Dr Jae Mie is a female doctor at Active Medical Centre who has worked in the medical field for over 7 years.
Dr Jae Mie believes in building strong relationships with her patients to provide them with long term continuity and a model of care that focuses on preventative health. With a special interest in Dermatology and an emphasis on Dermoscopy, Skin Checks, Mole Removal and Skin Cancer Medicine – Dr Jae Mie provides complete skin health checks and minor skin excisions.
Dr Jae Mie has a natural ability and passion for Paediatric Care and has completed a Diploma in Child Health to further enhance her skills in caring for the health and wellbeing of children. In addition to this, Dr Jae Mie has experience in family planning, reproductive and sexual health, cervical screening and implanon insertion and removal. Dr Jae Mie speaks English and Malay and enjoys playing the piano, painting and bush walking.
Dr Ummu Rishada Rauf

Dr Rishada is a female doctor at Active Medical Centre Caroline Springs who has over 8 years experience in general practice. With a Diploma in Child Health as well as Gynaecology and Obstetrics, Dr Rishada has a keen interest in paediatric care and women’s health.
Being a mother herself, Dr Rishada has a great understanding on how best to support parents and children to achieve positive outcomes for the entire family. Other special interests include mental health and post natal depression, obstetrics, gynaecology, emergency medicine, minor surgical procedures and immunisations, chronic disease, skin care, travel medicine, and aged care.
Dr Rishada can also provide patients with implanon and Mirena insertion and removal. With a zest for knowledge and a preventative approach to healthcare, Dr Rishada is here to support the health needs of the local community. Dr Rishada speaks English, Hindi, Tamil and Urdu.
Dr Sirisha Moparthi

Dr Sirisha has worked in the field of General Practice in the UK for over 8 years and offers holistic patient centred care to achieve positive outcomes for her patients.
With a special interest in Family Planning and Pregnancy Management, as well as Children’s and Women’s Health, Dr Sirisha has been awarded a Diploma from the Royal College of Obstetricians and Gynaecologists, along with a Diploma from the Faculty of Sexual and Reproductive Health in London.
Dr Sirisha speaks English, Hindi, Telugu, Urdu and is a busy mum who enjoys spending her free time with family and friends.
Dr Hormuz Parekh
Dr Hormuz Parekh is a male GP at Active Medical Centre, Caroline Springs and has worked in the field of General Practice for more than 10 years. He has developed broad clinical experience in many aspects of healthcare.
Dr Parekh is passionate about helping to care for the health and wellbeing of all patients, so that they can enjoy happy and healthy lives.

Dr Alex Stepanov
Dr Alex Stepanov has worked in the field of General Practice for more than 15 years, having collaborated with health care providers in Melbourne’s west to achieve positive outcomes for patients.
Dr Alex firmly believes in providing a patient centred model of care in an empathetic and respectful way.
His special interests include men, women and children’s health, mental health, travel medicine, chronic disease management, preventive care and the treatment of Hepatitis B and C.
Dr Alex is fluent in English, Russian and Ukranian and in his spare time enjoys spending time with his family.
Dr Baseerat Qureshi
With extensive experience in holistic and preventive care in general practice, Dr Qureshi takes an active approach to caring for families in the local community.
Dr Qureshi’s special interests include women’s, children’s and family health and has thorough experience in acute and emergency medicine.
Dr Qureshi is a female GP who can offer consultations to patients in English, Urdu, Punjabi and Hindi
Dr Kim Tran
Dr Kim Tran is a male GP at Active Medical Centre, Caroline Springs who has been involved in the healthcare of the Western Suburbs for the past 10 years.
Dr Kim is here to listen and adopts a holistic approach to patient care. He offers a patient centred model of care in a respectful way to families in the local community.
Dr Kim is a current examiner for the Royal College of General Practitioners. Special interests include general family health and chronic disease management of conditions such as asthma, emphysema, heart disease, diabetes and thyroid disease.
Dr Kim speaks English and Vietnamese and is learning Mandarin. He loves to spend time with family and friends. He has an interest in IT and is a keen sports fan.

Nutritionist / Dietitian
Nalani is a qualified Accredited Practising Dietitian; Accredited Nutritionist and member of the Dietitians Association of Australia. As well as having an accredited ‘Masters of Dietetics’ qualifications through the University of Queensland, Nalani’s credentials also include a Bachelor of Science from La Trobe University and a Graduate Diploma in Nutrition from Deakin University.
Nalani is experienced in a range of clinical conditions and has a particular interest in chronic disease management via the patient centered team care approach. Nalani has a special interest in chronic disease management, cardiovascular disease, dyslipidaemia, diabetes, CKD and weight management through diet and lifestyle modification.
With a Bachelor Degree in Occupational Therapy, Nermein has worked in Vocational Rehabilitation for many years and managed a team of allied health professionals across Victoria and Tasmania in both private and public sectors. Nermein has special interests in assisting patients with Chronic Medical conditions with high care needs and hand therapy, including the application of Orthotic devices such as splints in upper limb therapy.
Nermein enjoys working with people from diverse cultural backgrounds including refugees and asylum seekers and uses a client centred approach in her practice; working collaboratively with other health care practitioners to provide holistic interventions and achieve best outcomes.
Kiran is an Accredited Exercise Physiologist with an interest in improving quality of life, health and well-being through prescription of exercise. Kiran is specialised in graded exercise therapy and lifestyle interventions for people at risk of developing, or with existing chronic and complex medical conditions and injuries.
Kiran provides evidence based exercise programs that are specifically tailored to each individual client. He applies “exercise as a medicine” for the treatment of chronic diseases such as musculoskeletal, cardiovascular, Metabolic, neurological and other conditions.
Janet Yong is a Credentialed Diabetes Educator with over six years experience in Diabetes care, management and prevention.
Janet empowers patients to Eat Well, Move More and Love Life, and works with them to improve their diabetes management, offering culturally appropriate dietary changes, practical lifestyle modifications and optimising diabetes treatment through medication and/or insulin review.
Outside of consulting Janet is a facilitator for Diabetes Victoria. Playing an integral role in the free Life! program to help prevent diabetes for those at risk.
Dr Caelin Ling enjoys working together with patients to identify their unique goals and needs when helping them maximise their foot health and general wellbeing. With experience in community health, rural health, aged care and private practice, Dr. Ling is fascinated by aged care podiatry, dermatology, chronic disease-related foot health such as with diabetes, and the impact that podiatry can have on the lives of others.
Dr. Ling believes in ongoing learning, discovery, and improvement, both in and outside of his podiatric practice – using his spare time for travelling, sightseeing, film-making, cooking and volunteering with animal rescue groups to help find sheltered animals new homes.
